Hours:�37.5 per week - The service operates between 8am and 6pm Monday to Friday, but actual working hours will be determined by the needs of the service.
Leave:�30 days holiday per annum plus bank holidays (currently 8 per year). This is pro-rated for any employees working less than 37.5 hours per week.
Location:�Preston, Lancaster, Accrington, Blackpool, Kendal and Barrow

About the Role

HMP Resettlement Triage Workers will be required to deliver a professional, impartial pragmatic and outcome focussed Resettlement Service to offenders within all relevant prisons. This will be undertaken on site or via outreach to additional HMP locations within the CPA.

Resettlement Triage Workers will contribute to HMP Induction programme, completing BCST 2’s, working with Peers to identify prisoner needs, delivering immediate one off interventions and updating resettlement plans, as applicable.

Resettlement Triage Workers will be responsible for:

- Completion of BCST 2 for each offender
- Completion of Resettlement Plans within 12 weeks of pre-release
- Completing low level one off interventions
- Liaising with RO’s, other supply chain providers and prison agencies.
- Identifying required interventions and refer onto Shelter Resettlement Workers and other pathway providers
- Ensuring case details are entered onto the agreed Case Management systems (such as OASys, CRC systems and Shelter recording tool) accurately and punctually.

Some HMP locations are difficult to reach with the use of Public Transport. It is therefore preferable for candidates to have a full driver’s license and access to a vehicle with relevant insurance.

About the Service

Shelter are working across the North of England to deliver the new Transforming Rehabilitation Service.� Specifically, in Cumbria and Lancashire CPA the service comprises of a resettlement element, based in the four prisons and a community integration element based in the CRC offices across the Cumbria and Lancashire CPA.

About the Team

Position will be working within the TR Through the Gate Team. Based in one CPA:� substantive location at one prison but with an expectation to work across multiple locations within the CPA, as required to support cover and sickness.

The function of the team is deliver an integrated resettlement service, including, but not limited to, the following:

- Complete BCST 2 for all offenders within 5 days of BSCT 1 completion.
- Support Peers in the delivery of induction sessions for all offenders on reception.
- Deliver housing, finance and debt interventions via group and one to one appointments.
- Coordinate referrals to appropriate agencies to deliver additional pathway interventions.
- Complete pre-release planning, including review of agreed interventions, actioning any outstanding Areas of Concern and preparation for release.
